Immunization Tracker Instructions

How to view your immunization records and status in Immunization Tracker.

  1. Go to The NEST
  2. Choose “Student/Faculty Login”
  3. Use your Net ID and password to enter into the NEST system.
  4. Once logged in, click on the “Student Services” tab. Located at the bottom of the page, will be the “Immunization Tracker” link where you may choose from one of the following links:

    View Immunizations
    The page displays the immunizations on file with Health Services at the top, and your required immunizations at the bottom. The bottom section also indicates if you are compliant or non complaint. Requirements are based on the school in which you are enrolled

    Upcoming Immunizations & Tests
    The screen will display the due date for immunizations needed in the future.

    Enter Additional Immunizations
    Choose from the “Immunization/Test” drop down menu to select the immunization/test you are entering.

    Enter the date of the immunization date then click on the “Submit” button.

    If the selected immunization/test requires a result of Negative or Positive, an additional dialogue box will display where the result needs to be selected. After the date of the immunization/test and the result are entered, click on the “Submit” button.

Things to remember

  • This information is subject to audit. You may be required to submit documentation.
  • Immunizations should be entered in chronological order.
  • If you realize your immunization information is incorrect, you will need to submit a copy of your documentation to Health Services so it can be corrected.
  • Immunization Tracker will accept immunizations that are not required.
  • You may need to update the Immunization Tracker with immunizations that may expire in the future.
  • An official report of your immunizations can be requested from the Center for Health & Counseling, located in the Mike & Josie Harper Center.

Required Immunizations

All Students
Full & Part-time
Measles, Mumps & Rubella (MMR)

Two doses MMR vaccine given after the 1st birthday and at least one month apart are required of all Creighton students born after 1957.
      OR
Positive blood tests showing immunity for Measles, Mumps and Rubella are accepted.
History of illness does not meet this requirement.

* A registration hold will be placed on students that are not compliant.

All Health Science
Students

Dental, EMS, Medical, Nursing
Pharmacy &  Health Professions

Measles, Mumps & Rubella (MMR)

Two doses MMR vaccine given after the 1st birthday and at least one month apart.

This is required, regardless of date of birth.

DPT/Td/Tdap A three dose series-commonly given in childhood.

One dose of Tdap (Adacel or Boostrix) given after licensed in 2005.
Polio A three dose series-commonly given in childhood.
          OR
A positive blood test showing immunity for Polio is accepted.
Hepatitis B

A three dose series
           AND
A positive blood test showing immunity for Hepatitis B. (Hepatitis B surface antibody)

Varicella
(Chicken Pox)
A positive blood test showing immunity is required if student has history of chicken pox.
           OR
If no history of chicken pox, documentation of a two dose series.
Tuberculosis Screening Initial Screening
Two separate PPD skin tests within a one year period of time (2-step testing)
Annual screening
A  single PPD skin test annually after initial screening
Students with a history of a positive PPD skin test
Chest x-ray done within the past 12 months in the United States AND  
Annual Provider review
     OR
Annual QuantiFERON Gold testing also accepted
International Students
Measles, Mumps & Rubella (MMR)

Two doses MMR vaccine given after the 1st birthday and at least one month apart are required of all Creighton students born after 1957.

* A registration hold will be placed on students that are not compliant.

Tuberculosis Screening International Students will be screened upon arrival to campus unless documentation is provided for a negative PPD skin test done within the past 12 months.
PPD skin test required regardless of prior BCG vaccine.

Students with a history of a positive PPD skin test:
Chest x-ray done within the past 12 months in the United States.
Annual Provider review
QuantiFERON Gold testing also accepted 
Recommended
for
All Students
Hepatitis A Two dose series given 6-12 months apart
Students who travel abroad are encouraged to consider this vaccine.
Meningitis This vaccine is especially encouraged for students who will be living in resident halls for the first time. Persons traveling to endemic countries also need to consider receiving this vaccine.
Adacel/Boostrix
(Tdap vaccine)
A tetanus booster vaccine with whooping cough coverage is suggested before college.
Gardasil A three dose series for females only.

Please note all Immunizations are subject to Health Services Audit.
